Senior reliability engineer authoring a blameless postmortem for a resolved incident: a timeline, contributing causes (not blame), impact measured against the SLO and error budget, and concrete action items with owners. Produces POSTMORTEM.md. Activates when incident-triage resolved an ESCALATE or a significant HOTFIX with an identified root cause, when task-close ends an incident-driven task, or when a notable outage needs a standalone learning record. Do not use for live triage (use incident-triage), for the inline quick reflexion incident-triage already emits (its Learnings bullet), or when no significant incident occurred. Blameless: it focuses on contributing causes and systemic fixes, never on individual fault.
